Family Portrait Questionnaire

I'd love to get to know a bit more about your family before we take photos! Please take a few minutes to fill out this brief questionnaire so that I can learn about what makes your family unique (which helps me take photos that you'll love)!

How many people are in your family, and their names?

How old are your kids?

How would you describe your family's personality or "vibe" in three words?

Some ideas to get you started: Is your family spontaneous or structured? Quiet or outgoing? Bookish or athletic? Lively or laid-back? Or anything else you can think of!

Tell me about your kids. What are they interested in? Do they have any favorite toys or activities? Describe their personalities.

Is there anything you want to make absolutely sure we capture during your session? Do you have any specific expectations you'd like me to know about?

Another way to put it: What is your highest priority of the session—a perfect shot of the whole family? A sweet shot of just the kids? Candid shots? Individual shots? Of course we will try to get it all, but if you had to choose the one SURE THING.

Are you and/or any of your family members camera shy, and is there any way I can help you feel less nervous when we're shooting? Are there special things that make you and your kids smile or laugh?

How do you plan to use the images from our session?

E.g. framed on the wall, printed in a photo album, posted on social media, etc.
